I/We confirm that the metering installation at the property listed on this form is free of hazardous material such as asbestos. If metering technicians attend your property to replace or reprogramme the meter and are unable to do so because you fail to provide safe and easy access to the meter or your installation contains asbestos, you will be charged a call out fee of $147.
